Overview:


We are looking for dedicated individuals who can provide high quality direct care and support to residents within the community, focusing on personal wellbeing outcomes and individual wishes, to achieve strength-based support to those who need help to live independently again.

Full training will be provided and opportunities to progress within the service.

Hours:
Shift work between 7am to 10pm, 7 days a week. You will be required to work every other weekend as per rota.


We have a number of full and part time contracted hours and casual positions available across the county, including Buckingham, Aylesbury, Burnham, Chesham and Beaconsfield.


_ Please note, these positions are community based, not day centre based. Therefore a full UK driving licence is essential and having access to a vehicle. The post holder will be entitled to a car user allowance and a 45p per mile mileage claim allowance (HMRC mileage allowance)._

Despite the challenges that Adult Social Care has faced within the last 12 months, our approach in Buckinghamshire has not been to take shelter and wait for the storm to pass.

Instead, we have chosen to transform and restructure Adult Social Care so we can respond to a crisis as well as build for the future.

The Home Independence Team help residents stay in their own homes and become independent again.

If they've had a fall, a change in what they are able to do around the home or a spell in hospital, they may need help regaining the skills to do things that they used to do.


Home independence is short term support that will work with residents to practice everyday tasks, we provide a highly professional service to help them regain confidence and give practical support and advice.

About the role:


As a Social Care Assistant working within the Home Independence Team, you will be working out in the community to support residents that have been recently discharged from hospital or those who are living with a disability and need help to live independently and establish a routine to help them manage their lives day to day.

You will be supporting individuals to regain confidence and reach their goals of living independently again.

No two days will be the same as you meet a variety of people and support their individual needs. Some of the work you will carry out includes.

  • Providing people with information and advice in relation to care and support and assistance in accessing care and support through various means.
  • Working as part a CQC Registered service in the community, maintaining a high degree of autonomy and utilising own initiative in delivering compliant care and support independently.
  • Ensuring an agreed plan meets individual's care and support outcomes needs and to empower the individual to access early intervention and preventative services.
  • Informing senior colleagues and adhere to safeguarding procedure of any concerns which arise in relation to the safety of vulnerable adults.
  • Working in partnership with individuals, carers, service providers, voluntary sector and other professionals.
  • Providing personal care for individuals, this may include but is not limited to dressing, toileting, washing and general housekeeping.

On-the-go:


Our 'on-the-go' roles mean you'll usually start and finish your day from a base, moving around between different locations throughout the day.

You won't need a fixed desk to work from, but you'll have access to our network where you can keep up to date with admin and the latest staff updates.


About you:


Naturally, you will be a great people person, caring and empathetic as well as being able to deliver practical care and support to an agreed plan.

You will use your own initiative to problem solve and utilise remote management oversight, where required.


You may need to undertake a specialist role that requires specific training, such as Autism services or undertaking and administering medication.
