FLOATING MENTAL HEALTH SUPPORT WORKER

We pride ourselves on being specialists in the provision of support services to providers of supported housing for vulnerable people.

We are looking for people who can demonstrate that they:

- are experienced in working with vulnerable people


The key aspects of the role include:

  • Full time role for a fixed term of 3 months
  • 40 hours per week, 9am till 5pm.
  • Rate of pay £
    11.95 per hour.
  • Floating support worker based in the community. Providing support surrounding maintaining their mental health and enabling them to successfully resettle into the community. This role will work alongside the NHS through the 'Long Term Plan and You' initiative. This focuses on the longterm mental health and social care needs of the population alongside partner organisations.
  • Support all individuals on the caseload through one to one meetings and group settings, ensuring comprehensive assessments, risk assessments, and support plans are completed are completed to a high standard.
  • Build strong and sustainable relationships with internal and external stakeholders and organisations. Some of these include but are not limited to; Homeless Prevention Officers, Local Landlords, Housing Benefit Teams, Temporary Accommodation and Housing Associations, Hostels, Rehab Facilities, and other relevant organisations.
  • Undertake regular supervision with SIG operational management and other relevant persons to provide, when required updates on clients who have been transferred to primary care for additional support.
  • Attend and contribute to blended triage and other meetings with partner organisations to support individuals to achieve their personal goals, in line with their support plan.

Enhanced DBS Certificate less than 3 years old (Essential):


  • Experience of working in health, social care, voluntary and community sector environment (Essential):

Experience and knowledge of Mental Health agenda and working with mental health services, social care, voluntary and/or recovery focused services (Essential):

Job Types:
Full-time, Fixed term contract

Contract length: 3 months


Salary:
£11.95 per hour


Schedule:

  • 8 hour shift
  • Day shift
  • Monday to Friday

  • Mental health supported housing: 1 year (required)

Licence/Certification:

  • Enhanced DBS (required)
  • Driving Licence and access to own car (required)
