Description

The Community Independence Service provides locality based multi-professional rehabilitation, reablement and social care assessment and intervention for people over the age of 18 who primarily have a physical impairment.

We are looking for a Rehabilitation Assistant towork with people within their own homes and care settings who need help to stay independent.

This entails a comprehensive holistic assessment and may also involve:

Individual goal focused rehab planning
Provision of appropriate equipment and minor adaptations to support independence, or advice about selfpurchase
Falls assessment, education and prevention
Provision of appropriate equipment and minor adaptations to support independence, or advice about selfpurchase
Support to access a Personal Budget/Council managed service
Advice on Wellbeing and health
Signposting to other community services

**The service works to help people to achieve their goals and to remain as independent as possible.
