Description
A good opportunity has arisen for a Primary Care Mental Health Practitioner.

This role (which is jointly funded by the Primary Care Network (PCN) and ELFT) will be embedded in this new team structure, and represents an exciting opportunity to accelerate the transformation work underway across East London, while also bringing additional capacity to the new care models for those with serious mental illness, complex emotional needs, and disordered eating.


You will support local GP practices to help rebuild and reconnect healthcare team across the communities they serve, and will provide targeted care according to the needs of the population by supporting the PCN clinical strategy.


As a registered Nurse, you will be expected to work with some autonomy within this system, but would also connect into professional support structures already in existence in ELFT.

You would also be expected to work closely with other roles as part of the PCN's core multi-disciplinary team to help address the potential range of biopsychosocial needs of service users with mental health problems via personalized approaches.


  • To promote and offer a range of clinical and nonclinical interventions within the PCN Mental Health Team
  • To provide assessment and care planning expertise relevant to your profession
  • To contribute to the ongoing development and transformation of the care system
  • To provide effective signposting to a range of health, social, voluntary sector and communitybased resources and services.
  • To work with service users to:
  • support shared decision-making about self-management;
- provide brief psychological interventions, where qualified to do so and where appropriate;

  • To, where possible, operate without the need for formal referral from GPs, including accepting some direct bookings where appropriate.
  • To support the PCN practices in order to achieve specific targets relevant to mental health as may be dictated by local and national contracts (DES, NIS. QOF)
  • To participate at practice and networkbased MDTs, share good practice and take part in training activities for other members of the MDT.
  • To work in GP practices and support the team in reviewing patients with Depression and Anxiety who are usually managed in Primary care only.
  • To attend relevant meetings and work collaboratively with colleagues to enhance patient care
