The Macmillan Accessibility Co-ordinator is an exciting opportunity to join the inpatient team at Mount Vernon Cancer Centre in a brand new role.

The post holder will work closely with cancer patients to identify social and support needs through the completion of electronic holistic needs assessments (eHNA).

The eHNA tool will help identify any areas of concern for the patient and signpost as needed. The post holder will assist with discharge planning to prevent increased length of stay and improve patient experience.

The role will include liaising with community and social services and working with the existing accessibility coordinators within Mount Vernon to ensure a seamless service across the cancer centre, particularly for vulnerable patients.


As part of the Multi -disciplinary team the post holder will contribute to service provision by providing elements of direct and indirect patient care and clerical work.

The post holder will be expected to facilitate and support care delivery, information giving and communication and coordination.


As well as being a member of the team at the cancer centre the post holder will also have the additional benefits of being a Macmillan post holder.

Duties will be varied and dependent on the support needed on a daily basis, by the nursing staff


In particular:

  • To support patients to complete electronic holistic needs assessments, identify concerns and support the implementation of the care plan.
  • To work closely with staff across the cancer centre and within the community to support the patients social and support needs
  • Receive patient calls and make pre planned outbound calls to patients to assess ongoing needs.
  • To support vulnerable patients and those with complex support and care requirements within the specialist cancer inpatient environment.

  • Maintain excellent communication with patients, relatives and members of the multidisciplinary team regarding all aspects of care, demonstrating a variety of communication skills in accordance with the client group. Ensure confidentiality is maintained at all times.
  • Triage incoming calls, and initiate appropriate response according to pathways, using good communication skills, basic clinical awareness and appropriate tools and procedures.
  • Create a referral pathway for referral of patients requiring assessment across the cancer centre.
  • Provide basic telephone advice and refer on or signpost to other sources of support.
  • Coordinate and complete the necessary eHNA assessments, appointments or investigations for patients as required.
  • Use patient feedback and facilitate patient involvement to ensure that services are patient centred and reflect the diversity of the practice population.
  • Make sure that people affected by cancer are aware that they are interacting with a Macmillan professional and know about the full range of resources and services available through Macmillan.
  • Facilitate information provision for cancer patients, enabling patients understanding of their condition and treatment, navigation of available services and access to information and support for self-management where appropriate.
  • Actively engage with Macmillan Cancer Support to contribute expertise and support the Macmillan Corporate Strategy.
