Description

To provide clinical and operational leadershipto the District Nursing Out of Hours Team,to ensure the safe and effective delivery of Adult Community Nursing.

To provide support and guidance to colleagues to assure that work is carried out in a multi-disciplinary way to promote the needs of clients and patients.

In addition the post holder will support the Senior Manager and Operational Manager, to ensure that governance systems and processes are adhered to and delivered within the area of responsibility of the senior practitioner.

Undertake holistic Assessments and support staff with patients with patients who have complex nursing needs

Assess individual staff members with regards to competency and devlepment

Take appropriate action to deal with emergency situations that may arise

Provide Clinical leadership with the Out of Hours Team

  • Carry a caseload appropriate to the role in line with banding and professional expertise and be accountable within their teams for the appropriate allocation of referrals/cases.
  • Support staff to manage those patients presenting with complex needs appropriate to their role in line with banding and professional expertise.
  • Communicate sensitive information to patients, relatives, staff and other professionals where there may be barriers to understanding when planning and delivering direct patient care / rehabilitation.
  • Act professionally and responsibly at all times and maintain your continuous personal development in order to maintain your registration with the appropriate registering body.
