Qualifications

  • Educated to GCSE or equivalent (GCSE Maths & English C or above)
  • Coaching/counselling qualification / experience or other relevant qualification/experience involving reflective listening skills
  • Completed a four day PCI accredited health coaching training course or be willing to complete one prior to taking referrals.
  • Completed Tier 1 level of practice/expertise for health and wellbeing coaches or willing and supported to do so
  • Full driving licence and access to own transport

General

Salary:
£22,000 to £28,000 per annum dependent on experience / part time pro rata

Hours: 30 to 37.5 hours per week, divided between the PCN member practices according to their agreed schedule.

Summary


We're looking for a motivated team player with a clear passion for providing personalised lifestyle support including mental health support.


This bespoke role has been developed across three GP Practices in LS12; Armley Medical Practice, Priory View Medical Centre and Thornton Medical Centre as part of our collective vision to deliver better health outcomes and education for people living with long term health conditions, both physical and emotional, who could benefit from making behaviour changes to improve their quality of life and outcomes.


You will provide health coaching for physical health conditions, using motivational interviewing techniques and shared goal setting, taking place over various sessions.

You will provide information on services and provisions that can help promote and sustain a healthier lifestyle and better self-management of health conditions.


As part of our PCN's population health offering, you will also focus on supporting a specific group of men who are at risk of escalating poor mental health, providing mental health support to empower this patient cohort to improve their mental wellbeing.

The job presents an exciting opportunity to engage and listen to patients and understand their strengths and assets.

You will provide health coaching and mental health support and enable people to build a sustainable package of support, involving lots of different partners, to maintain and improve physical and mental health.


You will play a pivotal role in supporting and motivating people by coordinating health and social care services including, GP surgeries and their broad range of staff, local voluntary sector services, community healthcare services, social care services, pharmacists, and housing.

You will ensure that all patients receive the person-centred, coordinated care that they need and empower them to take control of their healthcare needs.

Your interventions will be led and co-designed by the patients you work with, their carers and wider supporting services.


Along with the wider Primary Care Network multidisciplinary team and our specific Mental Health and Wellbeing Coach, you will be the service users' go-to person, and act as a key point of contact to prevent professionals asking the person to repeatedly 'tell their story'.


The post holder will be required to travel from practice to practice and to other venues as part of this role.


Main Job Responsibilities

  • Support people to coproduce proactive, personcentred care plans based on what is important to them including achievable goals. Use health coaching techniques to motivate patients to develop the knowledge, skills and confidence to achieve their goals.
  • Empower people to manage their own health and improve their health outcomes and support them in making changes in their health related behaviour.
  • Enable service users to manage practical tasks through a motivational and problemsolving approach.
  • Manage and prioritise a caseload, in accordance with the needs, priorities and support required by individuals in the caseload. Along with a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, because the person's needs are beyond the scope of the health and
