An exciting opportunity has arisen for an enthusiastic Diabetes Dietitian to join our Diabetes Team to develop our diabetes transition and young adult care service.


The dietitian will be part of the wider diabetes multidisciplinary team with a focus to improve standards of diabetes care especially in the age group of years.

We are looking for someone with a passion for diabetes to provide specialist support and education for patients and their families and other health care professionals.

The role involves working autonomously as well as within the multidisciplinary team at Sherwood Forest Hospitals NHS Foundation Trust. Training and support will be provided.

  • Support several clinics, including transitional MDT clinics years), diabetes young adult clinics years) and joint dietitiandiabetes specialist nurse clinics.
  • Provide support and education to people with type 1 and type 2 diabetes and support management in the use of insulin pump therapy.
  • Create/adapt structured education course to appeal to the younger patient,
  • Work closely with DSN to establish links with primary care and other community providers to identity different mechanisms to support young adults with diabetes in the community.
  • Take part in student training.
  • Capture data on quarterly basis to evaluate the service.
  • To ensure Clinical Governance is integral to service delivery within the specialty area.
  • To represent the Nutrition and Dietetic department and contribute to the development of the wider Therapy service within Clinical Therapies.
