Description

The post holder is a Physical Health and Wellbeing Coach who acts within their professional boundaries to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing.

The role will include working with patients to increase their physical activity in order to improve overall health.

The post holder will also increase patient ability to access and utilise community support whilst providing access to self-management education, peer support and social prescribing using their personal knowledge and skills or utilising those within the wider Wellbeing Team.


Health and wellbeing coaches take an approach that considers the _'whole person' _in addressing existing issues and encourages proactive prevention of new illnesses.

The aim is to achieve positive long-term best outcomes for patients.

The approach will be to appreciate patients' circumstances in a non-judgemental manner. This will result in the need for positive communication and negotiation skills that will support the patient's personal choices.


The role will allow patients to increase their physical activity and make positive lifestyle choices in order to increase their health & wellbeing, both physically and mentally.

The post holder will have sufficient knowledge to educate patients in the impact of increased physical activity on various long term conditions and pain, whilst encouraging realistic and safe goals.


Then post holder will provide support to patients through individual consultations, classes, and by assisting in facilitation of group therapy where appropriate, as well as by supporting other local organisations, such as Winchester City Council, local gym facilities, voluntary organisations and peer support workers to provide quality services.


The role will be an integral part of the general practice team working in each practice and across the PCN.


Essential Qualifications:

  • Level 3 or greater qualifications in rehabilitation exercise for patients with longterm conditions
  • Level 3 or greater in nutrition and weight management
  • Qualification and experience in delivering an exercise on referral programme
  • Level 3 or above in rehabilitation for cancer patients

Desirable Qualifications:

  • Educated to degree level or equivalent in a sports science

Job Types:
Full-time, Part-time, Permanent

Part-time hours: per week


Salary:
£23,000.00-£25,000.00 per year
