Care/Support Coordinator

Role:
We are looking for a Care Coordinator to join our team at Purelife Care services.

In this role, you will effectively and efficiently manage the delivery of high quality care services, ensuring that support Workers are matched to Clients based on the level and type of support they need, along with, when possible, being in close proximity.

Experience working as an Administrator/coordinator is essential.

You'll need to be proactive and adaptable, with excellent communication, administration, finance and minute taking skills and experience, along with experience of completing contract management data reports.

You will be working closely with our young people, building positive and professional relationships, whilst encouraging them into independence and adulthood with safeguarding being your top priority.

Requirements:


  • At least an NVQ level 3 in Health & Social Care or equivalent youth related qualification
  • Minimum of 1 year Admin experience or working within a residential / care setting
  • Excellent safeguarding experience
  • A recent fully enhanced DBS ( or be prepared to have one processed with us )
  • Ideally 1 year experience working as a care coordinator for a health care organisation.
  • Knowledge of OFSTED Compliance is essential.
  • Ideally experienced in conducting support assessments, support reviews and being able to motivate support staff.
  • Good experience of key working Looked After Children with complex caseloads
  • Good standard of written work and ability to complete key working reports, care plans and progress reports.
  • Excellent work ethic and passion for working to progress young people to successful independence
  • Good experience of Child Sexual Exploitation, Criminal Exploitation and County Lines. Including LAC / MISPER processes
  • Knowledge and experience of working of substance misuse / therapeutic intervention
  • Ability to manage challenging behaviour and engage with young people in care
  • Full drivers licence ( desirable )

Main Tasks and Responsibilities

  • To ensure that support is provided within a professional, positive and homely environment which positively integrates race, spirituality, gender, disability and sexual orientation and meets individual needs.
  • Take and distribute minutes of meetings as required.
  • Coordinate and attend staff meetings, staff support groups and service meetings.
  • Coordinate and prepare for all meetings at the service as required.
  • Assist in ensuring GDPR compliance throughout the service.
  • Adhere to health and safety compliance, ensuring timely and accurate recording keeping.
  • To establish positive relationships with the young people, assessing needs and working jointly in producing, implementing, delivering and revising individual support plans.
  • To work with young people individually and in groups with the aim of enhancing relationships, selfconfidence, social skills, managing personal behaviour and life skills.
  • To organise and participate in activities for young people.
  • To adhere to all reporting and recording procedures and ensure the production of timely reports and correspondence to a professional level.
  • To communicate effectively with other agencies and professionals, and when requested prepare written reports.
  • To be responsible for dealing with petty cash and other purchasing duties in accordance with established procedures and policies, ensuring that all petty cash dealt with is accurately recorded and reported.
  • To act as a corporate parent to the young people, providing guidance and support to the young people whilst maintaining professional boundaries at all times.
  • To work with a young person and ensuring key working sessions are carried out in a timely and appropriate manner.
  • To ensure all duties associated with running the home during a shift are completed to a high standard

Such duties include:

daily observations, log book entries, completing running records, updating weekly planners, key worker sessions, fire safety, health and safety checks and any other documentation implemented by the management team.


  • To ensure necessary liaison and communication takes place within the home and outside with families, other members of the business, education providers, other agencies and the local community.
