Full and Part-time hours considered (minimum 30hr per week) Open to Registered Nurses and Paramedics


We are seeking to recruit 1 B7 Rapid Response Practitioners, with the skills to care for patients who present with an acute illness or exacerbation of a long term condition to avoid unnecessary hospital admission.

Practitioners will be expected to demonstrate advanced skills in assessment, monitoring and delivery of complex care planning to patients/service users across Gloucestershire and to include onward referral into the Integrated Community and Specialist Teams when medically stable.
