'To Coordinate all functions within our Dementia Service.**Working Monday
- Wednesday 9am 4pm**•To be responsible for booking all venues ensuring they are within budget, have good access and are suitable for group events.
- To be the first point of contact for referrals
- Complete basic assessment form via telephone ensuring that we are data compliant and update our internal databases
- Pass assessment form to the service manager to check eligibility on NHS system
- Create groups on our internal database in conjunction with the Health & Wellbeing Coordinators
- Add clients to the individual groups on our database and create waiting lists where appropriate
- Liaise with Health & Wellbeing Officer with regards to taking payments for class attendance for our memory club classes
- Prepare register of attendees and pass to relevant facilitators/ Assistant each month
- Prepare feedback forms and relevant monitoring forms and pass to relevant facilitator at the beginning of each course
- Prepare resources/activities for classes and liaise with facilitators and assistants
- Notify facilitators of any cancellations
- Notify clients of any changes to activity schedules
- Update venue gantt chart
- Advise clients/ carers of class start date/ times and venues
- Arrange taster sessions for paid for services if appropriate
- Complete relevant training to allow for cover for facilitator absence
- Liaise with the service manager regarding non
- Liaise with Office Manager regarding supplies required for each group
- Ensure that classes for CST are well attended with a minimum of 6 clients per class
- Check eligibility for CST
- Identify new venues ensuring they are within budget and cost effective
- Research and create a resources folder on the shared drive so facilitators can access resources and information.
- Ensure that the coordinator provides facilitators with relevant resources to run their groups.
