Description
To share and utilise areas of specialist practice with peers and colleagues to inform and enhance patient care. To work with the management / operational team to contribute to the development and delivery of service. Identify and use information sources such as the Directory of Services (DOS) to support and underpin clinical decision-making. Provide health education and self-management advice during the consultation process.

Refer to the ACP Lead / Lead SCP / medical Lead as necessary for advice. Facilitate and actively participate in the continuous quality improvement audit process. Reflect as necessary on this process. To attend and contribute to organised meetings.

To follow gtd healthcares policies and procedures, i.e., the failed encounter procedure and the home visiting policy. Understand and embrace local pathways including use of ED for urgent and immediate needs. Effective Communication Actively communicate with all staff. Actively communicate with other service providers about patient care i.e., child protection issues.

Work within the governance agenda contributing to service improvement.

Reflect the diversity of needs in the local healthcare community by utilising all available resources to facilitate access to the service where there are barriers to communication and understanding.

Utilise advanced listening skills, probing and facilitative skills across a diverse range of calls Face to Face assessments, some of which are highly challenging due to emotive circumstances or patient aggression.

The post holder should recognise the importance of effective communication within the team and will strive to communicate effectively with team members, patients, and carers Utilises enhanced communication skills to negotiate and provide support to service users who may be non-compliant with recommended outcomes whilst adhering to the relevant gtd healthcare policies and procedures.

Management of Risk Maintain a healthy and safe work environment for self and colleagues.

Have responsibility and accountability for the escalation of any adverse incidents or events to the appropriate persons both internally and externally, whilst ensuring that the appropriate documentation is recorded, and that the ACP lead / operational lead is actively involved and communicated with.

Corporate responsibilities Work as part of the gtd healthcare team to ensure a positive and productive team environment is developed and always maintained.

Personal/professional development The post-holder will participate in any training programme implemented by the organisation as part of employment, such training to include: Taking responsibility for maintaining a record of own personal and/or professional development.

Prioritise, organise and manage care in a manner that maintains and promotes quality. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.

Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.

Attend or complete all mandatory sessions e.g.

Child Protection and BLS etc.

Clinical Duties Assess patient both adults and children need from a physiological and psychological perspective and plan care accordingly in a timely manner.

To work with members of the MDT and independently to assess, plan, implement and evaluate interventions/treatment for patients with complex needs and care for patients.

To work with members of the MDT.

Provide health education and promote healthy lifestyle for patients and carers. Provide clinical and professional advice.

Promote safe systems of work and identify areas where changes in Urgent Care / primary care would achieve a more effective outcome.

Ensure resources are used efficiently and effectively and all equipment used in the care of patients is appropriately maintained and used only by those individuals with the appropriate training.
