Carers Support and Outreach Worker - Lead for hospital discharge

Salary:
£28,263 per annum


Full time (35 hours per week) short term contract minimum 9 months

The role:

The post holder will work alongside the support and information worker,

registering and triaging Carers, offering first contact support.

The post holder will lead on the hospital discharge work, at times, working out of St Georges hospital alongside the discharge team and hospital social care teams, providing information and support to Carers navigating the hospital discharge process.


Skills and experience:

The successful applicant will have outstanding organisational and communication skills, be confident delivering training and outreach.

They will be able to manage conflicting demands with tight deadlines, be a key team player and committed to working with Carers and volunteers.


They will have excellent IT skills and be able to adapt to providing services both face to face and via virtual platforms, whilst maintaining a consistent service for Carers.
