SALARY:
£15.00ph


HOURS: 21 hours per week


LENGTH: 12 months

Purpose

To harness collaborative working with local Caribbean & African people and to support the transformation of the health and wellbeing, through lived experiences that underpin the 5 key areas, goals, and outcomes of Core20PLUS.

A Core20PLUS connector on this programme is an individual with a lived experience of an inequality.

They work in partnership with decision makers to ensure that primary care services are as good as they can be for those accessing those services.


This role recognises and values the lived experience and the importance of connecting that lived experience with those making decisions in primary care services.

The individual will be driven and have a passion to improve services and uses their voice to advocate for Black Caribbean & African people.

Role Specification

  • To utilise local knowledge in working with the Caribbean & African community to ensure barriers to access and other identified issues which might prevent people from accessing care are addressed.
  • To understand the needs of the population experiencing poorer than average access and outcomes from healthcare.
  • To connect health care service providers with communities to improve health with focus on the five health inequalities priorities in Core20PLUS.
  • To be the voice that identifies barriers and champions change for those experiencing the health inequalities.
  • To work with service users at place level across the health priorities, and influence where possible a reduction in health inequalities across other locally identified priorities for the group in focus.
  • Willingness to contribute to codesign and coproduction in the priority areas with local service users bringing direct experience of health inequalities.
  • To join up/work in collaboration and assimilate Core20PLUS goals with other community roles such as BME Forum Long Term Conditions Team, Social Prescribing Link Workers, and Cancer champions.
  • To build on and enhance existing local communitybased initiatives, that focus on the 5 key areas, goals, and outcomes of Core20PLUS.
  • Make links with existing groups and networks that may want to be involved including bottomsup approach to reducing health inequalities.

Requirements:


  • Lived experience of an inequality.
  • Representative of Caribbean & African population groups.
  • Live local in district of designated PCN and have good connections to


the communities in focus be a powerful voice for their communities - connecting (or already be connected with) local voluntary sector and community groups, or faith organisations.


  • Be willing to be supported and trained in understanding and communicating the Core20PLUS objectives and health interventions.
  • Engagement with local health systems and service users.
  • Adopt the necessary communication skills that facilitate engagement that enables coproduction of pathways to improve inequalities.
  • Willingness to hold local health and care partners to account and represent their community views or lived experience.
  • Use community development and participative practice approach to achieve the aims of the project.

Role Outcomes

  • Organise and facilitate seven community outreach health events over 12 months.
  • Coordinate seven health awareness workshops focusing on the 5 key clinical areas.
  • Conduct 70 individual health checks within the community.
  • Provide people with the information, guidance and support needed to empower themselves to make the best choices for their own health.
  • Increase trust of healthcare services within Black African and Caribbean communities and breakdown any stigma around accessing both physical and mental healthcare support.
  • Create a newsletter that will be distributed to members and residents throughout Croydon.
