Job Summary


A skilled and dedicated Patient Services Coordinator is needed to join our supportive and survivor-led Rehabilitation team at The Katie Piper Foundation.

A charity dedicated to supporting the lives of Survivors of Burns and traumatic scars.

This is an excellent chance to join a passionate and devoted team that helps to transform the lives of Burns Survivors through specialist and holistic rehabilitation, bespoke to each survivor's needs and goals.

In this role, you'll be the first point of contact for survivors and health professionals referring to our services. You will coordinate appointments, and activities and ensure survivors have a smooth pathway when accessing our services.

You'll build relationships with a wide range of people including survivors, their families, carers and health professionals.


Predominantly a home-based role but with the expectation to stay overnight at The Katie Piper Foundation Rehabilitation Centre situated at Rainford Hall, St Helens for up to 5 nights a month.


Main duties and responsibilities:


  • To work proactively within The Katie Piper Foundation rehabilitation team, ensuring a smooth pathway for survivors accessing digital and residential rehabilitation and services.
  • To support the Head of Patient Services and rehabilitation team with administration tasks to ensure the effective running of the service and the delivery of service to survivors of burns and traumatic scars.
  • To provide advocacy and support to survivors of burns and scars.
  • To manage the patient database.

- To provide in-person support to patients accessing The Katie Piper Foundation Rehabilitation Centre when required. This will include staying overnight at the centre for up to 5 nights a month.

  • To work flexibly to meet the needs of the services delivered.
  • To travel for training and team development days.

Patient planning:


  • Work closely with The Katie Piper Foundation rehabilitation team to capture all relevant survivor information, referrals, and enquiries on the database, ensuring all data has been captured ahead of survivors accessing the services.
  • To liaise with survivors, their families, carers, and relevant professionals.
  • To support survivors practically, including making transport arrangements for residential survivors, assisting with admin tasks, and coordinating appointments with providers on behalf of survivors.
  • To work collaboratively with partner organisations regarding admission planning, assessment days, clinic appointments and services.
  • To lead and coordinate relationships with external activity providers and other third parties, ensuring information is centrally captured including contact details and service provided, signed contracts, confidentiality agreements and comprehensive insurance, in line with GDPR guidance.
  • To participate in the survivor evaluations and assessment days. For residential survivors to include the coordination of the day.

Admissions:


  • When appropriate ensure the residential accommodation is ready ahead of an admission, ensuring the provision of a food/refreshment welcome pack in line with the survivor's dietary requirements.
  • Engage with all parties regarding admission plans, dealing with survivor concerns in a compassionate and timely manner.
  • When appropriate navigate survivors around the rehabilitation centre/site and provide information on residential cottage/hotel accommodation including safety and security procedures.

Rehabilitation pathway:


  • To liaise with survivors to ensure a clear understanding of the services they are accessing which will be agreed by the rehabilitation Multi-Disciplinary Team (MDT) as appropriate.
  • Where necessary assist in the survivors' daily activities on and offsite including transportation for food shopping and taking part in their activities.
  • Close liaison with relevant staff members regarding the survivor therapy timetable.
  • Act as a source of support and advocacy to burn survivors, ensuring clear professional boundaries are always adhered to.
  • To use The Katie Piper Foundation database and ensure survivor electronic files are up to date with activities and progress during their rehabilitation or engagement with services.
  • To be fully aware of risk assessments for thirdparty providers and potential risks for the individual survivors when undertaking activities. To feedback on any concerns to the line manager or Katie Piper Foundation rehabilitation team.
  • In preparation for discharge, planning with the survivor to ensure their safe return home and smooth transition to the digital rehabilitation service.
To attend training and CPD to benefit the delivery of services provided to survivors attending rehabilitation.
