Description
We are looking to recruit a Band 4 Pathway Coordinator.

The post holder will use the Trust's information systems to undertake comprehensive validation of patient's pathways within the Diabetes &
Endocrinology Directorate in order to meet trust and national targets such as 18 weeks and Cancer Targets.


The aim of this is to ensure information is accurate and to escalate issues or delays in the patient's treatment pathway.

The post holder will be the
point of contact for all administrative issues relating to patients' pathway of care.


D&E are a Directorate within the GRaDE Care Group, which has staff based at both Royal Hallamshire Hospital and Northern General Hospital.

The post holder will be required to work Monday to Friday, between the hours of 8AM and 6PM,

To monitor and validate all patients on the 18 week and cancer pathways.

To flag up patients who my potentially breach the National Targets, to their line manager.

Investigate data errors and correct pathway status as required.

To liaise regularly with Clinician and Directorate Managers in connection with the pathways.

Support Admin staff with guidance and training to support pathway management.
