The post holder will be an expert practitioner in the Diabetes specialism and will autonomously assess, plan and deliver evidence based care, education and support to people with Type 1 and 2 Diabetes, their families and carers.

The role involves covering community based clinics but in this bank position there is also the opportunity to also work remotely undertaking telephone and virtual clinics.


The post holder will be an expert in Type 1 & Type 2 diabetes and will work collaboratively with allied healthcare professionals to deliver structured education programmes.

Additionally, the post holder will be an integral part of our wider multi-professional team involved with the management of patients with diabetes and supporting other health care professionals in providing care to patients with diabetes in Primary and Community Care settings.


  • To provide an expert role within the Integrated Diabetes Service for patients with Type 1 & Type 2 Diabetes.
  • Assess, plan, implement and evaluate programmes of care for adults with diabetes, through hospital and home, developing individual care plans in partnership with the patient, their family and relevant others
  • Facilitate and monitor the management of diabetes patients across primary and secondary care, and identify other complex care areas requiring input
  • To support & educate Diabetes patients in a variety of settings.
  • To offer ongoing management and support to patients with Diabetes through effective care planning
  • To support staff caring for Patients with Diabetes in a variety of settings
  • To be an integral member of the Integrated Diabetes Service working with all members of the Multi-Disciplinary team.
  • In parallel with service development, have the opportunity to participate in prescribing. Reflecting the requirements for nurse prescribing principle areas, knowledge and competencies required to underpin the practice of prescribing.
  • The post holder will be experienced in insulin pump therapy including those patients on "closed loop" therapy and be ablet o offer support to patients already using insulin pumps. The post holder will be willing to take part in regular pump MDT meetings.
  • The post holder will be experienced in starting patients on Flash Glucose Monitoring and in the interpretation of data.
