Due to further funding from NHSE, we now have vacancies for 3 Heart Failure Specialist Practitioners to work within the heart failure/virtual ward teams for 12 months.
Our heart failure service is looking for a highly motivated practitioners to join our very enthusiastic and busy team.
This is an ideal opportunity to develop skills and interest within the heart failure speciality and to gain experience for further development.
We have 3 posts available - 2 posts based at Royal Derby Hospital (South) and 1 post at Chesterfield Royal hospital (North).
The aim of these posts is to improve inpatient care for patients with heart failure and reduce length of stay and hospital readmissions.
We also have an integrated, tele-health team, using technology (telehealth), to support patients and have a dedicated Ambulatory heart Failure unit delivering parenteral diuretics and Iron therapy for heart failure patients along with cardiology virtual ward delivering hospital care at home.
This post is multi-faceted and involves working predominately within the Royal Derby Hospital or Chesterfield Royal hospital
However, we also review patients in the community (home visits/clinics) at times.
Within the acute trust, you will be reviewing patients who have been admitted to hospital with heart failure symptoms. The role works provides in-reaching to hospital wards, within the virtual ward and ambulatory service (south)
We aim to prevent avoidable admissions and support patients admitted with heart failure and support early discharges.
We work closely with the cardiac physiologists for device management and have weekly MDT's with the heart failure cardiologists at Royal Derby Hospital and monthly MDT's at Chesterfield Royal hospital.
We also link in with other specialities for MDT's so ap art from working closely with the cardiologists and physiologists we work closely with the respiratory, renal, palliative care and endocrine teams to support patient care.

It is Trust policy that for posts that are for a time limited period, applicants who are currently employed by an NHS organisation will only be appointed on a secondment basis and should have approval from their line manager this is possible before applying.

Patients are reviewed face to face within the patients' home and clinic settings and virtually via telephone and video consultations*.
Within Royal Derby Hospital, you will provide active management to patients requiring IV diuretics within the ambulatory HF unit, review patients that have been admitted to hospital and review patients at home following early supported discharge.
Provide specialist advice, support and education to other health care professionals in the management and treatment of heart failure.
