Central Cheshire Integrated Care Partnership is looking to recruit an enthusiastic individual due to the expansion of the administration team.

The Integrated Placement of Care team coordinates discharges from hospital by providing high quality care to patient's within their own home or a rehabilitation setting within the community.

The role would involve working alongside other administrative staff as well as multidisciplinary teams within the organisation.


The post holder will provide comprehensive, quality, confidential administrative support to the Integrated Placement of Care (IPOCH) Team employed by the Trust.

The Operational Service Assistant will be expected to organise their own workload and co-ordinate their activities in conjunction with, and supervised by the IPOCH coordinators, to ensure that an efficient service is provided.

The post holder will be capable of the full administrative function within the team, and be able to step in to fill gaps, to ensure continuity of services.

It is essential that the post holder is able to exercise initiative suitable to the role, and is fully supportive of other colleagues within the team.


  • To provide comprehensive administrative support to the IPOCH team and associated specialist services including managing calls, referrals and supporting the clinical staff providing short term intervention in patients homes to achieve the trust's aims of a 'home first' approach for patients.
  • Exercise own judgement based on acquired knowledge and experience, as to when to deal with queries personally, and when to involve other team members in the IPOCH Team and with what degree of urgency.
  • Build and maintain constructive relationships with others, within the department and externally in order to improve the effectiveness and efficiency of services to patients and support signposting where appropriate of queries and referrals.
  • Use of multiple IT based systems to support scheduling of visits and documentation including Malinko, EMIS & award winning gateway systems.
  • A significant part of the role is supporting the General Nursing Assistants, who support patients in their home environment delivering high quality care, promoting independence for those undergoing rehabilitation & providing highly dignified end of life care to service users & their families at a difficult time.

The post holder will provide administrative support which covers a 7 day a week service, including bank holidays.


Communicate factual information verbally or written to patients/carers, other team members and colleagues in the wider health and social care community including district nurses, therapy teams, advanced community practitioners and social workers in a timely manner.

Good telephone manner.

To maintain accurate personal and professional records adhering to trust policies and procedures.
