Job Title:
Community Care Worker


Responsibilities:


  • Provide direct care and support to individuals in the community
  • Assist clients with daily activities such as bathing, dressing, and meal preparation
  • Administer medication as directed by healthcare professionals
  • Monitor and report any changes in clients' physical or mental health
  • Assist with transportation to medical appointments or social outings
  • Maintain accurate and uptodate documentation of client care and progress
  • Collaborate with healthcare professionals and family members to develop and implement care plans

Qualifications:

  • Compassionate and patient demeanor
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Reliable transportation and valid driver's license
