Length of the contract: 6 months
Grafton Recruitment are now seeking to appoint an experienced Senior Nurse Assessor.
The Senior Nurse Assessor will act as a Case Manager for individual clients who are receiving continuing health care funding, to assess the needs of the client and their family to facilitate, plan and evaluate service provision to ensure the client achievesan optimum level of function and appropriate levels of care.
Duties of the Senior Nurse Assessor:
- To undertake Nursing Assessments for patients with identified health needs, ensuring they are robust and transparent.
- To provide advice, screen and coordinate assessments against the National Framework for Continuing Healthcare and Funded Nursing Care within Nursing Homes and to assist in the implementation of the policies.
- To work closely with the Lead Nurse and assist with observing the care homes visited, identifying unmet needs, and encouraging improvement in care standards.
- To work in collaboration with partners and other professionals to ensure patients' health care needs are met appropriately and effectively.
- To identify clients who are at risk, act upon and manage any issues related to the findings and report any concerns related to client care following the assessment.
Role Criteria:
- Educated to degree level or equivalent.
- Registered Nurse with a current NMC registration or equivalent.
- Previous experience undertaking Continuing Care Assessments.
- Experience working with children, young people and adults with complex and longterm health needs, disabilities and those with life limited/palliative or end of life.
- Knowledge of clinical governance framework.
- Knowledge of the personalisation and SEND agenda.
- Understanding of Safeguarding issues and policy framework.
- Understanding of Social Services and other Local Authority functions.
- Experience undertaking audits and implementing outcomes in a range of settings.
- Ability to demonstrate sensitive management of client and family/carer expectations.
- Excellent report writing skills, demonstrating sound clinical knowledge.
- Excellent interpersonal and communication skills.
- Ability to work without supervision, managing and prioritising own workload.
