Senior Practitioner - Adult Referral and Safeguarding Team

Kent

Up to £40 ph Umbrella

About the job:


As a Senior Practitioner in Adult Referral and Safeguarding, you will play a critical role in ensuring the safety and well-being of vulnerable adults within the community.

You will be responsible for managing and overseeing referrals, assessments, and interventions to protect adults at risk of harm or neglect.

This role requires strong interpersonal skills, sound judgment, and the ability to collaborate effectively with various stakeholders.


This is a locum contract where you will benefit from hybrid working

As a Senior Practitioner you will have the following responsibilities:

Key Responsibilities:

Referral Management:


  • Receive, screen, and triage incoming referrals concerning vulnerable adults.
  • Prioritize and allocate cases based on the urgency and severity of risks identified.
  • Liaise with external agencies and professionals to gather necessary information for assessments.

Assessment and Planning:


  • Conduct thorough assessments to evaluate the needs and risks faced by vulnerable adults.
  • Develop comprehensive care and support plans in collaboration with service users, families, and multidisciplinary teams.
  • Ensure that care plans are personcentered and comply with relevant legislation and policies.

Intervention and Support:


  • Provide direct support and interventions to adults at risk, coordinating services and resources.
  • Advocate for the rights and interests of vulnerable adults, ensuring their voices are heard and respected.
  • Monitor and review interventions to assess effectiveness and make adjustments as required.

Safeguarding and Risk Management:


  • Implement safeguarding procedures and protocols to protect adults from abuse, harm, or exploitation.
  • Conduct risk assessments and take appropriate actions to mitigate risks and safeguard vulnerable adults.
  • Collaborate with law enforcement and regulatory bodies as necessary in safeguarding investigations.

Collaboration and Leadership:


  • Work collaboratively with internal teams, external agencies, and partner organizations to achieve positive outcomes for vulnerable adults.
  • Provide leadership and guidance to junior staff and support their professional development.
  • Participate in team meetings, case conferences, and training sessions to enhance practice and knowledge.

Documentation and Reporting:


  • Maintain accurate and uptodate records of assessments, interventions, and case progress.
  • Prepare reports and documentation for legal proceedings, reviews, and audits.
  • Ensure compliance with data protection regulations and confidentiality policies.

Requirements:


  • Bachelor's or Master's degree in Social Work, Social Sciences, or a related field.
  • Relevant professional registration and licensing (e.g., Social Work England).
  • Proven experience in adult safeguarding, ideally in a senior practitioner or similar role.
  • Strong knowledge of relevant legislation, policies, and procedures related to adult social care and safeguarding.
  • Excellent communication skills with the ability to engage effectively with vulnerable adults, families, and professionals.
  • Critical thinking and decisionmaking abilities, with a strong ethical framework.
  • Ability to work under pressure, manage competing priorities, and maintain professionalism in challenging situations.
  • Commitment to continuous professional development and improvement of service delivery.
  • 5 years of satisfactory references
  • Driving License with access to a vehicle
  • SWE Registered
