To be the Professional Secretary of the Trusts Formulary and Medicines committee supporting the local implementation of NICE TAs and update of local formulary.
Liaising with the ICB to work together in aligning where appropriate the formulary across the ICS.Evaluate new drug request that come to Formulary & Medicines, support with robust critical appraisals of the new drug, and ensure completion of correct paperwork for submission.
Lead on Non-Formulary requests across the Trust.
Co-ordinate pro-active horizon scanning as part of the formulary planning and act as the Lead interface pharmacist with the ICS.
Provides expert advice on pharmaceutical matters for the division and undertakes risk management and ensures compliance with Medicines Legislation and other local and national policies and guidance.
Responsible for the development, implementation and review of policies, protocols and related guidance within the associated division to enable the safe, clinically effective and cost-effective use of medicines.
To support directorate pharmacists in management and approval of Shared Care guidelines.
Provide training support for other pharmacists to ensure that they can prepare business cases of sufficient quality to meet local needs.
Support clinical audit of medicines use and the development and update of guidelines, policies and other tools to implement alerts, formulary decisions and prescribing standards.
Support the ICS with its work on prescribing improvement programmes, medicines optimisation, a Greener NHS etc. Support the investigations of medicines related critical incidents and complaints and facilitate systems to implement best practice.When appropriate liaise with GPs and community pharmacists / PCN pharmacists and other hospitals to maintain continuity of care across the interface.
Line Management of specific 8as within pharmacy clinical team including Managing performance, sickness and recruitment processes as required.Exact area of responsibility to be agreed depending on expertise of post holder. Support co-ordination of high-cost drug audits to provide assurance that prescribing guidelines are being followed. If required support the Trusts Committee for Patient Group Directives (PGDs).
To provide clinical pharmaceutical care if required to wards and the pharmacy dispensary when appropriate, ensuring the effective and timely supply of medicines for in-patients.
Supervises the dispensing of medicines within legal and ethical requirements and local guidelines to ensure that patients receive the correct medication in a timely manner.
Participates in dispensary cover for up to 4 hours per week (10% of working time) to maintain basic pharmacy services and personal competency.
