Are you interested in helping older people to fulfil their goals and reduce their sense of social isolation? We are recruiting a team of enthusiastic and dedicated volunteers to be a part of the Community Companion team, helping to connect with patients who have been discharged from Kingston Hospital and/or Teddington Memorial Hospital who live alone in their own home within Kingston Borough.

This role extends the existing Discharge Support Service into people's homes and local communities. Supporting patients who have recently been discharged, you will help (re)connect them with their hobbies, interests, and independence.

This will give you a chance to get to know an elderly person over an extended time period of - 6 months and help them achieve their goals for living well in their later years.

Following on from their weekly telephone calls, you will meet with the patient.

Working with them to create meaningful life goals for staying safe, well and supported following their discharge home from hospital.

What this role will involve (These will vary depending on the patient, and may vary from session to session with the patient):

  • Making visits to patients' homes in Kingston Borough Provide information about local support services or groups that might meet patient's needs
  • Going out for a walk together (a route or area where the walk will take place will be agreed upon beforehand)
  • Attending a local event together
  • Helping with access to hobbies and interests, e.g. light gardening e.t.c.
  • Accompanying a patient to the shops or out and about in their local community
  • Encouraging and supporting with social activities, e.g. going out for coffee

Essential Information:


Please kindly note, you will be given a 20-minute assessment on the week commencing Tuesday 19 September 2023 at the Volunteering office.

The Mandatory training for this role will be on Tuesday 24 October 2023.

  • EU national identity card
  • EU only

Group Two:


  • Driving Licence photo card (UK, IOM or CI)
  • Current EU Photo Driving Licence
  • Driving licence oldstyle paper licence (must be UK)
  • Bank statement (UK/EEA) less than 3 months old
  • Utility bill less than 3 months old (UK)
  • Utility bill 2nd Supplier less than 3 months old
  • Council Tax Statement issued in the last 12 months
  • Mortgage statement (UK or EEA) must be a recognised lender and issued in the last 12 months.

NB:
If you are aged years, in full time education and you are unable to provide two documents from group 2 above, the following ID options can be used:

  • Full UK birth certificate (A4 size original, not photocopy, containing mother and fathers details)
  • Letter from head teacher or college principal, confirming you are a student and containing your name, current address and date of birth, PLUS an official exam certificate in your name and a National Insurance number letter.
