Adult Clinical Lead

Thornbury Community Services (part of Xyla, within Acacium Group)

Field based role with frequent travel across the South of England

Permanent, full time Monday-Friday

Salary:
£42,000-£45,000 DOE + 5% annual bonus + £3,000 car allowance

In this role you will oversee the bespoke care and support of clients, helping them to achieve their full potential and experience an increased quality of life.

You will support the Senior Clinical Team in the assessment process and setting up new complex care packages, as well as carrying out patient centered care plans.

Every day you will...

  • Undertake initial holistic nursing assessments in partnership with the service user, their family and healthcare workers, as well as developing individualised care plans to ensure goals are patient focused, realistic and achievable
  • Be accountable for the production of standard and bespoke risk management plans for clients, ensuring any identified high risk is escalated and acted upon, working with other stakeholders to plan, monitor and review effectiveness of any plan in place
  • Be accountable for the delivery of client specific clinical training for staff within packages where required and delegation of tasks
  • Ensure all clients allocated are regularly reviewed in accordance with company policies, contributing towards the implementation of CQC regulations, auditing documentation and escalating concerns

To thrive in this role, you must have...

  • A RGN registration with a current NMC pin, band 6 or above
  • Experience in either complex care, critical care, ITU, HDU or A&E
  • Recent experience with tracheostomy and ventilation, being fully competent in both areas
  • A passion for delivering excellence in clinical and holist
