Who Are We Looking For?
As the Care Coordinator, you will be responsible for the overall management and scheduling of client rotas and will participate in the on-call rota.
We are looking for someone who is willing to lead by example and go the extra mile for their team, creating a work atmosphere that promotes a high quality of work life.
You will be instrumental in creating and maintaining a culture of performance and excellence, ensuring safe and effective care is provided to all clients.
Flexibility to the needs of the service, the ability to multitask, and performing well under pressure are key aspects of this role.
Your main responsibilities will include:
- Managing and scheduling client rotas efficiently
- Regular communication with carers and clients
- Delivering highquality care and support to clients
- Participating in the oncall rota
- Assisting in the completion of audits, spot checks, and subsequent action plans to improve business processes
- Conducting service user assessments and care plans
- Liaising with local authorities when required
- Supporting the Registered Manager
Please note that although our office hours are 9AM to 5PM Monday to Friday, there will be an expectation to work flexible hours.
Qualifications and Experience
- A relevant qualification, e.g., NVQ at level 3, such as Health & Social Care (Adults)
- At least 1 year of experience as a Care Coordinator (desirable)
- Experience with Access People Planner and Access Care Planning (desirable)
- Extensive experience in domiciliary care
Salary:
£23,000.00-£26,000.00 per year
Schedule:
- 8 hour shift
- Monday to Friday
