A DAY IN THE LIFE OF A REHAB ASSISTANT PRACTITIONER

  • What you'll be doing..._
Reporting to the Rehab and Wellbeing Team Lead, you can expect your working day to include the following:

  • Working as part of a well-established forward thinking and friendly therapy team, you will work alongside the wider hospice multidisciplinary team to facilitate high-quality person-centered holistic care.
  • The role is varied, with a diverse caseload, supporting people in the inpatient unit, in their own homes or in the Netley Centre. No two days will be the same
  • You will manage your own caseload day to day, working independently and flexibly, with support from qualified therapy colleagues as required.
  • You will support people to access a varied and meaningful rehabilitation programme, aimed at maximising independence and wellbeing.
  • You will carry out functional assessments and provide skilled rehab interventions focused on patient centered goals, empowerment, enablement, and selfmanagement.
  • Utilising the dedicated Rehab Gym, you will support people to carry out planned exercise programmes.
  • With the support of therapy colleagues, you will lead on the delivery of therapeutic group work and dropin sessions.
  • Utilising the dedicated Art Room facility, you will promote the use of creative activities as a therapeutic media to maximise quality of life (training is provided).

To succeed in the role of Rehab Assistant Practitioner you will need the following qualities and skills:

  • A people person with excellent communication skills, who loves encouraging and motivating others.
  • Takes great pride in what they do and passionate about making a difference.
  • A creative problem solver who loves working as part of a team but is selfmotivated.
  • Has experience of working with patients with complex healthcare or rehabilitation needs and high levels of technical competence.
  • Experience of working within the community, in partnership with other agencies would be a distinct advantage.
  • You will need access to a vehicle which will allow you to travel as required.
  • Hours of work will be 22.5hrs per week, ideally over 3 days.

Salary:
£25,914 - £28,187 (pro-rata)

Holiday: 35 days per annum (including public holidays) (pro-rata)
