Description
We are looking for a health link worker to join our Bank team. You will work with the homeless or people rough sleeping across Somerset.


You will need to work as part of a team developing a service to provide early intervention and prevention nursing support for people who are experiencing homeless or rough sleeping across Somerset.

This includes those in hostels and supportive accommodation.


You will play a vital role tackling health inequalities by working with one of the most vulnerable groups in the county.

Working collaboratively with people and partner agencies, you will proactively engage with individuals at a range of venues in local communities, helping people to improve their lifestyle and overall health by contributing to a holistic approach that deals with the wider determinants of health.

With a Clinical Lead and a team of experienced nurses you will support appropriate and compassionate health care.

You will work with the clinical lead and nurses to ensure that the team develop to meet the needs identified and undertake any relevant training required to enable you to work as an autonomous practitioner.
