Description
The Clinical Lead will be required to manage a team of staff who cover acute and community sites.

You will operationally supporting wards and the Integrated Discharge team to discharge patients once they no longer meet the Criteria to Reside.

Support patient flow by following escalation processes for all patients that require support on discharge via Discharge to Assess for pathways 1-3.

Work closely with all providers within One Herefordshire to ensure that patients are discharged safely and efficiently.

Ensure data is accurate to support the escalation process
