ALocal Authority is looking for
Hospital Discharge Social Workers. These are all
PERMANENT positions that are mostly
full-time with some potential for
part-time.
This
Local Authority is based in
South Yorkshire but operates hybrid working (3 days in the office and 2 days at home and vice versa - dependent on diary) meaning that you could live outside of the region but be able to feasibly work for them.
You will need to be a
Qualified Social Worker with
post qualification experience and a knowledge on working within Hospital environments ideally.
outbound patients.

Your responsibilities as the Hospital Discharge Social Worker:
- Completing D2A Assessments
- Working towards KPIs
- Undertaking Face to Face clients visits
- Caseholding
- Multiprofessional working
- Completing Safeguarding Assessments
- Evidence based Practice
Requirements of you as the Hospital Discharge Social Worker:
- A recognised Social Work qualification (Degree/DipSW/CQSW)
- A valid Social Work England registration
- Experience of Safeguarding
- Must hold a full UK Driving Licence
Location:
South Yorkshire (hybrid working)
Salary Scale:
£29,447 - £39,513
