Children's Nurse CYPCC (Children and Young People's Continuing Care)

Venn Group is currently working with a large NHS company who are looking for a Children's Nurse CYPCC. Below is a brief synopsis on the role;

Duration: 3 Months Initially

Location:
Remote mainly - Usually the team go onsite for training and team building 1 or 2 times a month but might change to 1x week Based in Windsor

Rate:
Inside IR3- PAYE £ £32.43 = £301.12 OR £337.85 Umbrella

Hours: 37.5

Be a key member of the CHC and Children' Continuing Care team as well as supporting effective communication and stakeholder management, both internally and externally.
Work with the individual and their family to identify care needs and support in care planning for patients with a Personal Health Budget (PHB).
Undertake specialist nursing assessments for children with complex needs in relation to care packages, respite care and placement for Continuing Care with regards to specialist children's continuing care issues.
Support in the development of a range of care packages for children with complex health needs including palliative care services.
As a Nurse Assessor you will be responsible for completing assessments and reviews of continuing care packages ensuring children and young people are receiving high quality, safe and effective care and provide expert advice to other professionals to providecare.
Providing excellent quality DSTs for submission
Experience working with children care team, panels and doing reviews
Experience in completing CHC assessments and DST
RCN qualified
Experience using patient systems (e.g. Broadcare, Caretrack)
Ability to work in a team
Ability to work in a fastpaced environment and under pressure
