Main Responsibilities In accordance with service requirements, you will: Provide treatment to patients in the community, including, but not limited to, wound management, suture removal, health checks, and disease monitoring.

Work with patients to optimise the management of long-term conditions, including Blood Borne Viruses. Encourage smoking cessation and signpost patients to relevant service or support, where appropriate.

Take every opportunity to encourage patients to make small, manageable changes to reduce the risks associated with their current behaviour.

Support patients to access other services they may need to help improve their health. Manage / support with medical emergencies in the community, when necessary.

Provide other clinical duties within your skill set, as required, following the Make Every Contact Count approach to your care.

Work independently to provide the care to the patients allocated by the Primary Care Outreach Nurse.


Develop relationships with a variety of other organisations across the region, including, but not limited to, Stockton Council and the VCSE sector, in order to maintain an awareness of the relevant available services.

Work closely with colleagues from H&SH, CGL, Public Health Stockton, and other shareholders, where appropriate, to ensure a secure professional service for the patients.

Keep up to date with current, evidence-based approaches to patient care and service delivery.

Reporting Due to the nature of lone working, it is required that you keep in touch with the Nurse Lead during the shift by phone or calling into the office.


Record work activities, complaints, errors, concerns, and other untoward occurrences to the appropriate person, in line with policies, procedures, and standards.

If required, participate in clinical governance and clinical audit activities to ensure the continual improvement of service delivery. Communication Always present yourself in a manner that is open and honest.

Work in partnership with colleagues including the Primary Care Training Hub to enhance your own confidence, competence, and best practice.


Support patients and colleagues, both in your team and in other organisations, in a polite and respectful manner, using appropriate language and behaviour.

Attend team meetings, briefing sessions, and all other relevant forums. Professional Development Keep up to date with all mandatory training to ensure compliance and competence. Attend training courses in house, online, and externally, to develop expertise and further knowledge.

Take responsibility for updating, documenting, and maintaining your own knowledge and skills framework, and sharing professional development with H&SH. Express needs or desire for further training and work within own remit at all times. Participate in appraisals, performance reviews and professional development plans on an ongoing basis with H&SH when required. Special Working Conditions Because of the nature of this role, there is an element of lone working in the community.

You will be provided with a mobile phone and a Reliance protect device to use for your role.

You will be allocated a workload by the Primary Care Outreach Nurse and will have access to the resources required for the role (dressings, equipment, etc.).
