This is an exciting opportunity to be part of a dynamic and diverse team, supporting individuals with complex health needs within Lincolnshire

The post holder will deliver comprehensive and co-ordinated specialist assessment, review, case management and monitoring to individuals in accordance with the NHS Continuing Healthcare (CHC) Eligibility Criteria who fall within the ICB's remit to assess


This will involve working with individuals and their families and working in partnership with all stakeholders involved in meeting the needs of the local population.


The post holder will ensure that the health care needs of individuals referred for CHC are assessed and identified and met in accordance with the National Framework for CHC.

The post holder will also oversee case management to individuals who have been assessed to meet eligibility, review eligibility on a regular basis and ensure that the care continues to meet need ensuring the optimal use of NHS resources

The post holder will be responsible for the line management of case managers within the team.


The post holder will be responsible for ensuring all those who are eligible for continuing care are offered a personal health budget to meet their care needs.

To act on initiative for effective service delivery

To actively support the Continuing Healthcare Service in working towards the service's goals and objectives.

Required to maintain constructive relationships with a broad range of internal and external stakeholders.

To work closely with all care providers involved in delivering the service

Ensure the clinical team are maintaining data systems in line with standard operating policy and procedures.

To provide accurate and legible recording of information.

Deliver against objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines.

The post holder will carry out any other duties as may reasonably be required by their line manager.


The post holder will support the Deputy and Head of CHC Clinical services to ensure that their workstream is planned and managed effectively.

They will take a proactive role in supporting the successful delivery of overall Continuing Healthcare Services


NHS Lincolnshire Integrated Care Board (ICB), is a new statutory organisation bringing the NHS together locally to improve population health and establish shared strategic priorities within the NHS.

The ICB is part of a wider Lincolnshire Integrated Care System which has four key objectives:
Improve outcomes in population health and healthcare.

Tackle inequalities in outcomes, experience, and access.

Enhance productivity and value for money.

Help the NHS support broader social and economic development.


By working together with the ICB, the wider system will deliver its agreed vision of 'Better Lives for the people of Lincolnshire'.

Lincolnshire ICB will use its resources and powers to achieve demonstrable progress on these aims, collaborating to tackle complex challenges including:

Improving the health of children and young people.

Supporting people to stay well and independent.

Acting sooner to help those with preventable conditions.

Supporting those with long-term conditions or mental health issues.

Caring for those with multiple needs as populations age.

Getting the best from collective resources so people get care as quickly as possible.


Allocation of case managers for all eligible individuals, to ensure close working and collaboration with care managers to ensure healthcare needs are met and the individual is safely and appropriately cared for.

Monitoring in line with the National Framework will ensure the package of care meets the individual's required needs

Responsible for the validation of cases

Support individuals and their representatives to contribute to and be confident in the CHC process by encouraging their participation in a transparent and equitable process; ensure this is accurately documented as part of the assessment

Demonstrate ability to complete documentation or enter data accurately, consistently and in a timely manner

Ensures credible evidence is documented fully to support decision making and later review of the case through audit and/or independent review

The post requires highly developed skills of assessing, planning, implementing and evaluating care. Accuracy is important. Good observational skills are essential.


Responsible for the commissioning of safe care and case co-ordination of individuals who are funded and cared for in a variety of settings e.g.

ventilated patients cared for at home, patients with severe and enduring mental and physical illness, rapidly deteriorating conditions which require very close monitoring and a responsive service to meet their needs.

A small percentage of these individuals are placed out of county and in order to manage these care packages, travel outside of Lincolnshire may be necessary.
