    Clinical Aged Care Assessor Opportunity

    We are seeking a highly skilled Clinical Aged Care Assessor to join our team in New England. As a key member of our Assessment Services team, you will be responsible for conducting comprehensive assessments to identify the complex needs of clients and referring them to appropriate aged care services under the Aged Care Act.

    Key Responsibilities:

    • Conduct accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services.
    • Collaborate with clients, families, and multidisciplinary teams to develop and implement individualized care plans.
    • Maintain currency of knowledge relating to aged care services, including funded and non-funded options.
    • Assist with continuous improvement frameworks to evaluate and enhance assessment processes and outcomes.

    Requirements:

    • Tertiary qualification in a health-related discipline directly related to Health, Aged Care, or a related specialist area.
    • Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA).
    • Australian Driver's license and access to a registered, insured vehicle.
    • Minimum of one year demonstrated experience in Australia or overseas delivering services in Aged Care settings and/or to Aged persons, including those living with dementia.
