Description

The service operates 24 hours per day, 365 days per year to cover our 8am-8pm 7 days a week model.


Shortlisting is planned for: 29 March 2023


Interviews are planned for: 5th April 2023

The post holder will have continuing responsibility for the management of a defined caseload. They will be responsible for the continuity of care and be the lead person for communication to identified stakeholders.

The primary function of the role is to provide proactive, responsive services that address the anticipatory and maintenance care needs of the patients/clients within the defined caseload.

This will be done via a holistic assessment of care needs and the provision of clinical care for patients/clients with acute and chronic highly complex needs.

CHC Experience

Wound / pressure Ulcer / Leg Ulcer comps

Catheter experience

Palliative experience / Syringe driver experience

Experience in report writing

Supporting staff with development

Experience of supporting complex patients

Good time management and organisational skills

Provide robust holistic assessments to patients with complex needs

Work with the wider MDT

Caseload management

  • Interpersonal communication skills, relevant to student learning and assessment
  • Conducting objective, evidence based assessments of students
  • Providing constructive feedback to facilitate professional development in others, and
  • Knowledge of the assessment process and their role within it.
  • Have an understanding of the proficiencies and programme outcomes that he student they assess is aiming to achiev
