A new opportunity has arisen for a Care Home Care Coordinator to join our team as part of the Picton Primary Care Network (PCN) NHS.
This is a hugely exciting opportunity to work innovatively across our network to support the growth of our Care Home and Housebound services.
Main duties of the job
The Care Home Care Coordinator will provide administrative and leadership support to our Care Home and Housebound team.
They will be responsible for liaising with our Care Homes to coordinate reviews, as well as work creatively and collaboratively to develop and lead new services and projects that improve patient care.
They will be required to communicate effectively and sensitively to patients, clinicians and practice admin staff across the Network GP practices.
Additionally the Care Home Care Coordinator will be responsible for service evaluation by analysing data and reporting to the Care Home and Housebound team and Network management.
About us
Picton PCN consists of 6 GP practices that cover 45,000 patients.
We have a large Network team that consists of Pharmacists, Physios, Paramedics, Social Prescribers, Care Coordinators, Mental Health Nurses and a Network Manager.
We work together to provide quality care by delivering clinical appointments in our GP practices and health events in the community to our patients.
