Description

Dementia Support Hampshire & IOW is a small independent charity that provides emotional support, information, and signposting for people living with dementia and their families in Hampshire and the Isle of Wight.

We do this by hosting peer support groups in the heart of the community and via our helpline.


The main duties of the role will be to co-ordinate our existing peer support groups and open some new groups in new locations across South/West Hampshire.

The groups are held every afternoon Monday-Friday.


Responsibilities:


  • Effectively plan to ensure the smooth running of our existing and new peer support groups
  • Coordinate and manage existing volunteer teams and recruit, train and manage new volunteers ongoing
  • Educate and inform individuals about available resources and services
  • Collaborate with internal and external professionals to coordinate access to services to ensure the best possible outcomes for individuals
  • Advocate for individuals and help them navigate the health & social care system
  • Organise day trips, outings, events and activities for group members
  • Update our CRM (database) to ensure all actions and outcomes are recorded
  • Update social media for your groups and activities.

Experience:


  • Good problem solving and decisionmaking skills and the ability to prioritise and plan your work
  • Lived experience of dementia through caring for a loved one would be an advantage, or experience of working with/caring for people living with dementia or vulnerable adults
  • Strong communication skills, both written and verbal, to effectively interact with individuals and their families, community health professionals, community partners, staff and volunteers
  • Understand the importance of data collection and the need to evidence impact, as well as experience of using a CRM (database)
  • Ability to work independently as well as part of a team
This position requires a compassionate individual who is dedicated to making a positive impact on the lives of others. The Peer Support Coordinator will play a vital role in helping individuals access the support they need to thrive.

Use of own car is essential. Mileage expenses will be covered.


Able to carry activity equipment from group to group and assist in setting up tables and chairs at each location.
