Description

To coordinate the delivery and development of the Critical Care Outreach service through the use of effective evidence based practice in line with agreed national standards for patient safety and monitoring.

To support and improve services for level 1 patients on the ward at risk of deterioration, and in need of level 2 or level 3 care.

To provide an integrated Trust wide approach to Critical Care with the provision of the service outside the boundary of the ICCU.

To enable timely discharges from ICCU by supporting the continuing recovery and rehabilitation of patients transferred to the ward.


To provide highly specialised interventions and support with appropriate physical assessment, observation and treatment for acutely / critically ill patients on the ward.

To be accountable for the planning and delivery of safe, effective and compassionate patient care in relation to dedicated pathways of care, working as an autonomous practitioner.

To continuously improve clinical care standards in line with the clinical governance framework. To exercise accountability as set out in the NMC Code of Conduct.

To ensure effective communication internally and externally regarding patient care and resources. Provide education and training to clinical staff across the Trust.

Provide specialist clinical advice and support to patients and clinical staff within the Trust on the management and care of patients within the specialist area.

To represent the service nationwide.
