Duties:
- Conduct assessments to determine clients' needs and develop individualized care plans
- Provide counseling and emotional support to individuals and families
- Connect clients with community resources, such as housing assistance, healthcare services, and financial support
- Collaborate with other professionals, such as doctors, psychologists, and educators, to coordinate care for clients
- Advocate for clients' rights and ensure they receive the necessary support and services
- Monitor clients' progress and make adjustments to care plans as needed
- Maintain accurate and uptodate documentation of client interactions and progress
Requirements:
- Degree in Social Work or related field
- Certification as a Social Worker (if applicable)
- Strong communication and interpersonal skills
- Ability to assess client needs and develop appropriate care plans
- Knowledge of community resources and services
- Experience in supervising or mentoring other social workers is a plus
