Duties:

• Conduct assessments to determine clients' needs and develop individualized care plans
• Provide counseling and emotional support to individuals and families
• Connect clients with community resources, such as housing assistance, healthcare services, and financial support
• Collaborate with other professionals, such as doctors, psychologists, and educators, to coordinate care for clients
• Advocate for clients' rights and ensure they receive the necessary support and services
• Monitor clients' progress and make adjustments to care plans as needed
• Maintain accurate and uptodate documentation of client interactions and progress

Requirements:

• Degree in Social Work or related field
• Certification as a Social Worker (if applicable)
• Strong communication and interpersonal skills
• Ability to assess client needs and develop appropriate care plans
• Knowledge of community resources and services
• Experience in supervising or mentoring other social workers is a plus
