An exciting opportunity has arisen for a Band 6 Junior Sister/Charge Nurse to join our Children's community outreach team. The team is looking for a confident, compassionate and clinically credible nurse to join our dedicated team.

The post holder will provide high quality, evidence based paediatric care using a family centred approach and effective continuity of care for the benefit of the children and their families in the community by giving practical expertise, support and specialist advice through close liaison with primary health care professionals, under the direction of the Children's Outreach Team Leader.


You will be a highly motivated and innovative nurse leading our team with the support of the team leader, helping to deliver high quality care to our patients.

You will demonstrate excellent knowledge and previous experience in Community Paediatric nursing.

You will be working collaboratively with other MDT members, in addition to being confident in working under your own initiative in order to prioritise your clinical care and workload accordingly.

You will be approachable and supportive of your nursing team and will be pro-active with projects on the ward.


You will support the Team leader with all aspects of management and leadership, promoting a positive patient experience and supporting the Quality Improvement Programme.


  • To act in partnership with the child and family. To assess patient's needs and implement and evaluate patient care in accordance with the wishes of the family and in close liaison with the primary health care team.
  • To provide support and reassurance to children and their families in order to increase their confidence as carers and prevent avoidable admission to hospital when possible.
  • To act as a resource to other professionals within the primary health care team and to provide paediatric advice or teaching to children and their families to ensure their knowledge, understanding and competence to carry out procedures in the home.
  • To provide training sessions as negotiated for members of the primary health care team or to patients, their family and carers.
  • To work within national, regional and local guidelines relating to the provision of continuing care of children
  • To maintain clinical expertise and develop research based practice skills within the scope of professional practice.
  • To support the team leader in monitoring and controlling revenue expenditure so that budgetary limits are not exceeded.
  • In the absence of the team leader, to take responsibility for the day to day running of the service
  • To work with the team leader to ensure effective communication between all staff and patients
