Hours per week: 37.5 hours per week (Full time). Part time hours considered


Salary:
Agenda for Change Band 7 (pro-rata for part time)

Annual Leave: 30 days (based on full time hours, pro-rated for part time) plus Bank Holiday entitlement

Job Summary:


As an experienced Physician Associate, you will work as part of the Primary Care Network (PCN) Team in a number of areas and using a wide range of skills and competencies to support our GP practices to deliver care and support to the Surrey Heath population and to reduce the workload of the local GPs.

This may include, but is not limited to;

  • Providing holistic assessment, diagnosis, offering advice and guidance, and initiating treatment (where appropriate and within competence)
  • Supporting GPs with managing investigation results
  • Supporting GPs with managing their clinical correspondence


As an experienced Physician Associate you will have a minimum of two years' experience and act within your professional boundaries, providing first point of contact care for patients from initial history taking, clinical assessment, examination, investigation, diagnosis, treatment and evaluation of their care under the supervision of a GP.


You will work as part of the PCN clinical team, embedded within the teams in you nominated GP practices (normally two practices) to support the delivery of high quality care and relevant service specifications.

There is also opportunity to work in conjunction with the PCN Care Homes Team.


You will demonstrate safe clinical decision-making and expert care for patients within the general practice, supported by and in consultation with a duty GP.


You will work with the multi-disciplinary general practice team to support the delivery of policy and procedures and meet the needs of patients.

Supervision and mentoring will be provided by the appropriate senior medical staff - the type of support will depend on the post holder's skills and knowledge and determined by our clinical governance arrangements.

You will have annual appraisals and continuing professional development as set out by the Faculty of Physician Associates (FPA).


Surrey Heath Community Providers offer a 365 day, 52 week service delivery and as part of that we offer a 7 days a week from 0800 hrs - 2000hrs service.

So you may be required to work evenings, weekends, and Bank Holidays as part of your normal working hours, as per the needs of the business.


Key Responsibilities and Duties:

To deliver a high standard of patient care using advanced autonomous clinical skills with in-depth theoretical knowledge and evidence based practice working under the supervision of a GP


To manage a clinical caseload and deal with presenting patient's needs in a primary care setting including supporting the practice to deliver the Primary Care Network Direct Enhanced Service, Local Commissioned Services and QOF (Quality Outcomes Framework).


To provide clinical leadership where appropriate within the PCN Care Homes Team or practice(s) and support other members of the team to develop and maintain clinical skills appropriate to your role and expertise.

Initially the role will ensure a breadth of clinical work, telephone, video and face to face.

Focus will be on the registered population, from cradle to grave providing an holistic approach to high quality care and service delivery.

The role will be based across two surgeries, on the day support will be available at site and via your GP lead.

The exact model will be determined by the needs of the nominated GP practice(s) and may mean undertaking care home rounds and visits or providing general capacity within the GP practice to release a GP to undertake the care homes work.


CLINICAL

  • Provide highquality direct clinical care to patients / residents using established clinical guidelines, delivered in a timely manner.
  • Carry out consultations (whether face to face, online or phone), with patients of all ages as an autonomous practitioner and using own clinical judgement to diagnose, treat, refer and/or discharge patients.
  • Identify, signpost or refer patients at risk of developing longterm conditions, preventing adverse effects on the patients' health, making referrals within locally agreed protocols, guidance and pathways.
