_Would you like to be part of an exciting new pilot project to enable positive hospital discharges into care homes?_

We are looking for driven, determined assessors with social care experience at a senior or management level to work on behalf of social care providers: facilitating rapid, accurate assessments to ensure people who are medically fit for discharge can be settled in a care home as quickly and smoothly as possible.

We understand there is a need for more detailed, accurate and up to date assessments to enable care homes to accept new admissions from hospitals and that the best person to ensure this will work in, or have experience of, receiving assessments and admissions from a hospital setting.

Salary:
£30,000.00 per year
