JOB PURPOSE:

The main purpose of this role is to lead the development of integrated operational delivery of adult health and social care in the specified locality to deliver better outcomes for people to live as independent lives as possible.

The postholder will be directly responsible for the implementation, direct management and delivery of the Integrated Neighbourhood Team (INT), working alongside other operational managers and specialist services.

This will also be a key role to lead the strategic development in individual localities to support and progress integrated working with partners and communities to ensure the service offer to local people is joined up and providing the best quality and evidence-based care.

You will be responsible for leading on specific clinical/care governance activity within the INT, which will include the oversight and monitoring of standards, responding to complaints, staff support, learning, development, supervision, and the implementation of policy and clinical guidelines.


As an experienced manager of complex change and multidisciplinary teams, the Integrated Locality Lead will be expected to deliver transformational as well as operational change to ensure the priorities of the Alliance partners and of the locality are delivered effectively.

The post holder will work closely with transformational colleagues and professional leads to deliver these changes.

You will be expected to promote the Alliance vision, objectives, and priorities effectively to your staff, our partners, and the public and always inspire others by role modelling organisational values.

You will promote a culture of value for money, outcome-based customer / patient focus and continuous improvement.


KEY TASKS:

Shaping and leading integrated working Lead the development of the INT using the maturity matrix as a guide ensuring the core principles of a one team approach is progressed, removing duplication and streamlining policy and practice.

This will include developing single referral pathways, integrated priority triaging and trusted assessment, shared access to records and shared care planning Develop a shared leadership approach with social care/health care in the locality that supports integrated working and removes silos in delivery.

This will lead to single leadership of the multidisciplinary team across health and social care, working towards pooled resources and shared decision making Provide local leadership support to the development of a place-based improvement plan that supports the Alliance Delivery Plan working closely with system partners, local communities and colleagues from Transformation and Professional Leadership.

This will include using data and collaborative working with locality partners and services to inform priority setting and planning for changes in delivery and utilisation of capacity Lead the improvement in joint working practice with mental health and primary care in the locality and create the multidisciplinary team approach to case management, review and care coordination Develop the interface with the INT and local communities ensuring alternatives to formal care are supported by other local networks and groups.


This will include an improvement in utilisation of social prescribing Support the system cultural shift to a vision of support to people in their own home rather than a system of a separate organisations.

This means supporting development of 24/7 support, access to specialist advice and further development of digital technology for remote monitoring and self-managed care Lead effective partnership working within and outside of the organisation, creating a culture that promotes meaningful participation of individuals, communities, families and carers and co-production of services and solutions Create and sustain an organisational environment where people feel supported to challenge on issues of discrimination and oppression Take strategic responsibility for developing and sustaining a culture where the principles of social justice, social inclusion and equality are applied to strategic decision-making Take responsibility for continuously developing my strategic leadership and my ability to have influence and impact on my local and wider systems Prevention, proactive care and population health management approach Support the Alliance ambition to implement a proactive holistic model of care that identifies early the people most vulnerable and/or at risk Work towards implementing a phased approach towards population health management making use of tools already in use to facilitate case finding.
