What you'll do:


We are looking for someone with an inspiring and creative personality to work as part of a team within the Independent Living Medication Support Service.


You will work with clients diagnosed with a mental health condition and who are living within the borough of Lambeth in their own homes.

The role of the Specialist Support Worker is to enable our customers to develop greater independence and fulfilling lives.

As part of your role, you will be required to oversee Medication compliance, provide practical support to those with general needs issues.


Create robust Support and Risk plans, updating as required, and working with a patch list of customers on an agreed basis.


Enable customers to improve their overall wellbeing, through maintaining good physical and mental health, motivating customers to part take in social activity, and supporting customers to identify goals to improve their quality of life.


Ensure customers have applied for the correct benefits; that customers are managing their rent and bills, whilst maintaining their home to a good standard.


Applicants require a good standard of English and Maths, and established IT skills in the workplace, to support the multiply IT systems we use within the organisation.

We are looking for staff members who are motivated and keen to positively engage with our customers.


About you:


Medication Observance:

Support people who are struggling with medication concordance between the hours of 8am to 8pm 7 days a week

Complete home visits to observe customers take medication in the borough of Lambeth

  • Brixton, Streatham, West Norwood, Kennington, Clapham, Oval, and Vauxhall
Able to lone work in the community

  • Using available forms of transport to complete home visits.
Support people who are stepping down to more independence. This could be from registered to lower support accommodation; people returning to their homes after prolonged hospital admission.

Support to identify barriers to medication concordance, to identify and achieve goals which may be towards medication self-management.


To increase the level of contact with people in their homes whose main requirement of support is connected to their medication to feedback to clinical team and other support providers progress with medication and other support needs and to quickly alert clinicians to any concerns.

To connect individuals, as appropriate, with other opportunities of social inclusion and support as part of the Living Well Network

To support the development of a more collaborative discussion with customers on how they are supported to receive medication.


Key Support Working:
