Background


The Cornforth Partnership is an award winning charity with over 25 years' experience of providing support and opportunities to residents living in deprived and left behind communities across County Durham.

Its mission is to "provide the right support in the right place at the right time to enable the development of healthy, vibrant and aspirational communities".


The partnership has been successful in receiving funding from the National Lottery Community Fund to work collaboratively with 4 other charitable organisations in County Durham to tackle poor health and improve employment opportunities.


The Durham Together project is a new consortium approach to addressing the needs of those most deprived communities within County Durham and providing local people with opportunity to improve their personal circumstance and put them at the forefront of employment opportunities.


Job Purpose


Community Coaches will work with local people and communities to help identify and take practical steps to overcome barriers affecting their health and wellbeing and support them to take up identified training and employment opportunities which are realistic and sustainable.


You will support a caseload of clients eligable to the service and work with them in community venues to develop and implement actions and solutions to barriers which are negatively affecting their health, wellbeing and employability.


You will actively promote the Durham Together project to a wide range of stakeholders and be key to ensuring deprived communities in County Durham are not left out and have ample opportunity to participate.

You will empower, motivate and actively support clients to make and sustain positive change.

You will promote a flexible and High quality service reflective of participant and client need. Working in partnership with key partners of The Durham Together project and wider stakeholders.

You will be expected to work on your own initiative but be part of an integrated team.

Offering a person centred approach to mentoring, coaching and tackling barriers, providing outreach support in community venues across County Durham.


Key Roles/Duties

  • To manage the recruitment, retention, training and development of participants, building positive relationships and developing effective actions to overcome barriers as a pathway to positive change.
  • Hold individual responsibility and accountability for a caseload of participants using a positive enabling approach.
  • To work with key stakeholders, service delivery partners, statutory and non-statutory organisations in order to promote and generate referrals to the package of support available.
  • To provide a flexible and responsive service, developing comprehensive support plans based on client needs, continuously reviewing and reflecting with clients as appropriate.
  • Work in partnership with stakeholders and other intervention programmes to maximise collective support available for participants in addressing wider issues.
  • To ensure support is timely, client led and in line with Durham Together priorities and the core values and mission of The Cornforth Partnership.
  • Provide structures and smart information, advice, guidance and support.
  • Actively and positively promote the service to key stakeholders and the wider community.
  • Maintain accurate data and casefiles at the first and every subsequent instance of completion.
  • To be flexible in delivery and working hours including some unsociable hours.

Qualifications:

Educated to a minimum level 3 or equivalent

Level 3 information, advice and guidance.

Knowledge

Good, sound understanding of the needs of those living in deprived communities. Good sound knowledge and of benefit system and processes. Good understanding of County Durham and the services available within the area.

Lived experience of living/working in a deprived community. Working background of benefit support and IAG. An understanding of client led approaches. Experience of working collaboratively to achieve outcomes.

Experience

Extensive experience of proving mentoring support to achieve positive and substation change. Experience of working within the most hard to reach and most furthest away from support. Understanding of performance management and monitoring. Experience of working collaboratively to achieve change.

Experience of working with all sectors. Experience of outcome stars, HACT and SMART plan actioning. Understanding of UK SPF funding.

Attitude and Skills

Abilities to prioritise workloads and work on own initiative. Ability to communicate effectively partners, participants and staff on all levels. Excellent time management and ability to meet deadlines. A creative approach to engagement. Good I.T skills

Demonstrable customer care skills. Effective team skills. Ability to show empathy and share experiences.
