Description
An exciting opportunity has arisen within the Central Validation Team for a Patient Pathway Coordinators.


The roles occupy a central position within General Surgery & Colorectal services, enabling you to work across a range of multi-disciplinary teams in the management of patients' treatment pathways.

This includes guaranteeing the quality of data across a number of databases within the Trusts' Patient Administration System (PAS) to ensure that patients are in the correct place with respect to their treatment pathway, and that relevant outcomes are correctly recorded and communicated.


Excellent pathway management is fundamental to the patient experience and services rely heavily on this role to ensure that any trends or issues in patients' treatment pathways are highlighted.

Therefore, the post holder will be expected to assess, analyse and interpret data, and show excellent communication skills to maintain regular contact with the team and present any key findings.

Provide assurance of the accuracy and completeness of RTT patient pathways, and thus compliance with standards;
Ensure errors or omissions are identified and corrected;
Identify and minimise risk, waste and inefficiencies;
Ensure that staff have started/amended/closed pathways in line with the Elective Access Policy and SOPs, and identifies training or process change requirements where this is not the case;
Improve the efficiency and quality of patient care by ensuring that appointment outcomes, care plans and next steps are well planned and adhere to defined processes, and clearly communicated to patients;
Provides assurance of the accuracy of the Trust's reported performance against national operational standards;
Enables operational teams to manage waiting lists with confidence in pathway information and overall data integrity.
