Roles and Responsibilities Support General Practice to coordinate practice level Health Visitor Multi-Disciplinary Team invites monthly (minimum 6 a year) Ensure Health Visitor / Primary Liaison six to 8 weekly meetings are up and running according to NIS specification in each practice in Waltham Forest.

Work with the GP Practices to identify a representative to liaise with allied childrens projects.

Ensure updated GP bypass numbers is made available to Health Visiting (HV) Service Ensure Named HV for each practice circulated from THGPCG.

Identify/support and support practices who are struggling to organise practice level Health Visitor MDT meetings.

Liaise with Family Liaison Coordinator (working within Whipps Cross Emergency Department regarding Frequent Attenders (7 PCNs). Liaise with other stakeholders as necessary eg Health Visitors, Early Help, regarding frequent attenders or other ED concerns.

Attend existing PCN meetings to update the members of number of frequent attenders to ED from the PCN & numbers of children seen for 48hr asthma review and remind them of the HV/MDTs.

Regular liaison with the 48h review team (admin support for The Clinical Director) Liaise with specialist teams e.g.

NEL Asthma practitioner practice education and community specialist nurse re local training needs. Support immunisations coordinators with practice/PCN level issues focussing on variability.

Work with doctors re QI projects for LTCs e.g diabetes, epilepsy and sickle cell Quality assurance project surrounding Maternal and baby Postnatal check offer and compliance/uptake.

Proactively manage the co-ordination of the 6-monthly BCYP MDT learning events Assist collating information relevant to GP intranet with regards to BCYP.


To be available as a point of contact for other childrens stakeholders across Health and Social Care across Primary Care services.
