We are looking for an enthusiastic individual who can demonstrate they have experience of working with adults.

You will be required to work with adults and carers over the age of 16 who may present with a wide range of needs, and who will require a person centred and outcomes focussed response.

In this role, there will be a focus on supporting people who are in interim care placements to return to their own home.

The offer of an interim care placement is made when a patient is ready for hospital discharge, but the Health and Social Care Partnership is unable to provide an appropriate care at home package immediately, or when their first choice care home is temporarily unavailable.

You will be expected to give advice and practical help on the challenges associated with daily living activities.

You will have strong assessment skills and will be able to plan, maintain, and monitor packages of care, as well as have the flexibility and skills to respond to changing needs of individuals.

Ideally, you will be able to demonstrate experience of multi-disciplinary working and be committed to working closely with colleagues from other disciplines.

We can offer a wide range of opportunities for your continuous professional development. We provide supportive management and flexible working arrangements to help you achieve a healthy work/life balance.

This is an excellent opportunity to join a well established, highly skilled team, with excellent training, supervision and development opportunities provided.

Benefits also include flexible working scheme, modern working practices, generous annual leave entitlement and pension scheme.
