Discharge HUB Clinical Lead -Harrow Integrated Care Coordination Service (ICCS)


An exciting opportunity has arisen to appoint a Discharge Hub Clinical lead for the Harrow Integrated Care Coordination Service (ICCS).

The Post holder will play a key role in providing expert clinical and operational leadership in conjunction with ICCS team and partner organisations across all pathways in delivering safe and timely discharge from acute hospital ensuring quality care closer to home


The Discharge Hub Clinical Lead for Northwick Park Discharge Hub will play a key role in providing expert Clinical and operational leadership in conjunction with the Integrated Care Coordination Team Lead, Head of Clinical Services Harrow Integrated Care and partner organisations across all pathways in delivering on safe and timely discharges, ensuring quality of care.

The post holder will be accountable for the provision of effective professional leadership and pro
- active resource management for all relevant pathways. As a professional practitioner, the post holder will also be expected to act as a advocate for Community services in general, and CLCH in particular. To work with the Integrated Care Coordination Team Lead create an environment of continuous quality improvement and professional development ensuring that the delivery of care is regularly reviewed and that service developments are implemented when necessary.
